What I don't understand about this is why taking driving lessons would help you conquer the phobia. Surely, if you were already capable of driving, that is not something you'd forget. The problem would be in your mind, indeed as you say - a phobia, in this case amaxophobia. I can't see how extra driving lessons could have helped, particularly as you were a passenger when the accident happened so it clearly wasn't a result of any mistake you made.

Yes, i have a phobia. It is anthropophobia and it can make living quite difficult at times. However, living is difficult for most of us, isn't it? Compared to the billions in the Majority World, I consider myself to be very fortunate for they must contend with far more than I, simply to survive, let alone to live.
